Understanding Fertility Challenges
There are many reasons conception can take longer than expected or pregnancies don’t continue, and they’re not always detected by standard tests.
Underlying factors can include:

Functional Testing when Helpful
This may include hormone metabolites, nutrient analysis, thyroid function, vaginal microbiome sequencing, gut microbiome testing, and metabolic markers.
Functional Testing
Standard fertility investigations are an important starting point, but they don't always tell the full story. Where the picture remains unclear, functional testing can provide a level of detail that changes everything. Depending on your history and presentation, testing may include:

✔️ Hormone Metabolite Testing (DUTCH or ENDOMAP Test) A standard blood hormone panel tells you what hormones are present at a single point in time. This test goes considerably further, mapping how your body is producing, processing, and clearing hormones across a full cycle. This includes oestrogen metabolism pathways, progesterone output, cortisol patterns, and androgen activity that a serum test simply won't reveal. For women with unexplained cycle irregularities, recurrent loss, or luteal phase concerns, this level of detail is often where the missing pieces are found.
✔️ Complete Thyroid Panel
Thyroid function is routinely assessed in fertility investigations but generally only TSH is measured. A complete panel includes TSH, free T3, free T4, rT3 and thyroid antibodies. Subclinical thyroid dysfunction and autoimmune thyroid activity are both associated with implantation failure and recurrent pregnancy loss, and both are frequently missed by standard screening.
✔️ Vaginal Microbiome Sequencing
A healthy vaginal microbiome is predominantly Lactobacillus-dominant, and this balance matters more to fertility than most women (and many practitioners) realise. Disruption to this environment can affect sperm viability, increase infection risk, and alter the conditions necessary for conception and early implantation. Vaginal microbiome sequencing identifies the specific microbial environment present, allowing for targeted support rather than generalised treatment. This is rarely assessed in standard fertility care, but it is an area where the evidence is growing steadily.
✔️ Gut Microbiome Testing
The gut microbiome has a far-reaching influence on fertility, shaping immune regulation, inflammation, hormone metabolism, and nutrient absorption, all of which feed directly into reproductive health. Gut microbiome testing provides a detailed picture of microbial diversity and balance, and can identify dysbiosis, bacterial overgrowth, or other patterns that may be quietly disrupting the hormonal and immune environment needed for conception and pregnancy.
✔️ Nutrient Analysis
Nutrient testing in a fertility context is not simply about ruling out deficiency. It is about establishing whether your levels are optimal for the demands of conception and early pregnancy. Key nutrients I assess include active B12, ferritin, vitamin D, zinc, folate, and iodine, with reference to ranges that support reproductive function rather than simply the lower threshold of normal. We can also screen for MTHFR and other genetic SNPs that impact the way you metabolise nutrients. Egg quality, progesterone production, implantation, and early embryo development are all directly influenced by nutritional status in the months before conception.
Testing is never mandatory, but when the picture is complex, it can be the difference between guessing and knowing.
